WHO defines hypertension (HTN) as systolic blood pressure equal to or above 140 mm Hg and/or a diastolic blood pressure equal to or above 90 mm Hg.
Hypertension can be defined as a persistent elevation in blood pressure above the expected normal range. In 2011, the UK National Institute for Health and Clinical Excellence (NICE), working in collaboration with the British Hypertension Society, released updated guidance on the definition and management of hypertension.
Highlights:
DEFINING HYPERTENSION
Normotensive | Clinic BP <140/90 mm Hg or ambulatory BP <135/85 mm Hg |
Stage 1 hypertension | Clinic BP >140/90 mm Hg and ambulatory or home average BP >135/85 mm Hg |
Stage 2 hypertension | Clinic BP >160/90 mm Hg and ambulatory or home average BP >150/95 mm Hg |
Severe hypertension | Clinic systolic pressure >180 mm Hg or clinic diastolic pressure > 110 mmHg |

This chapter module covers the pre-op evaluation, optimisation strategies, anaesthesia-related concerns and perioperative management of patients with hypertension posted for non-cardiac surgery. Questions on ‘Hypertension’ are asked with HTN forming the as ‘sole-morbidity’ or as a part of ‘multiple comorbidities’ scenarios.
Candidates should know the definition and staging/grading of HTN, pathophysiology, anaesthetic implications of the disease, and the current guidelines on peri-op case management.
In this chapter, we have comprehensively covered the major concerns associated with the presence of hypertension in a patient posted for non-cardiac surgery with due regards to syllabus recommendations of the European/UK board syllabus for EDAIC/FRCA.
