Explanation:

  • The likely source of infection in the patient is streptococcus pneumoniae, a lancet-shaped encapsulated gram-positive diplococcus.
  • Found in the nasopharynx of 5–10% of healthy adults.
  • Diabetic, immunocompromised, asplenic/ hyposplenism patients are more susceptible to infections.
  • Typically, the onset is acute, with high fever, cough, and pleuritic chest pain and a chest x-ray show a single pneumonic patch. (as in the present case).
  • The polysaccharide capsule of the bacteria is the prime determinant of its virulence. Antibodies against the capsular polysaccharide promote the killing of the bacterium. The bacterium is usually penicillin-sensitive and the initial therapy should start with the penicillin group unless resistance is documented. Penicillin G is the drug of choice for Gram-positive diplococci (pneumococcal pneumonia). Penicillin-resistant strains are sensitive to macrolides, vancomycin, tazobactam–piperacillin (tazocin), and fluoroquinolones
  • Gentamycin is most effective on Gram-negative organisms. Other drugs that can be used are carbenicillin and ampicillin; however, they are not first-line. Ciprofloxacin is only moderately active against streptococci species.
